Registration is now open for our upcoming webinar on Wednesday, June 28 at 2pm!
Effective July 1, 2017, all OHA-funded Medical Case Management providers will be required to collect new data related to acuity, referrals, linkage to care, and clinical screening.
During this webinar, MDPH and JSI staff will walk through the new fields and supporting documentation. We will also share three new data reports that will: 1) allow providers to identify clients who have not had an acuity assessment in six months, 2) identify clients without CD4 or viral load tests in the past six months, and 3) make sure clients’ “annual review” information is updated every six months.
