Hello CAREWare MA providers,

If you are receiving this email, it means that you have not yet submitted a CAREWare MA user agreement for 2018. If you do not re-sign your user agreement by the end of the year, you will be locked out and unable to access CAREWare MA until a user agreement is in place.

All CAREWare MA users must re-sign user agreements before January 1. For security reasons, this is an annual requirement. Users with expired user agreements will be locked out effective January 2, 2018.

A copy of the user agreement is attached. Please complete one user agreement for each CAREWare user, and return it by fax to 617-482-0617 c/o Ashley Hatcher. If you do not hear from JSI within 72 hours of submitting a user agreement, please email the helpdesk at CAREWareMAhelpdesk@jsi.com.
